Certification Sponsorship Application

A completed copy of this form must be on file in the AFA Office at least 60 days prior to the Certification Date.

If the Applicant does not have insurance through the AFA, the application must also include a Certificate of Liability Insurance in the amount of $1 million naming the AFA as an additional insured.

Please Note: If you are holding your Certification on private property (i.e. someone’s private barn or shop), proof of insurance must be provided to have your host application approved. Certifications taking place on commercial properties (large farms, fairgrounds, vet clinics, equestrian facilities, schools, etc.) are covered under our AFA insurance policy and do not require proof of insurance.
